IV Cannulation: Step-by-Step Guide


A clinical reference for performing peripheral IV cannulation safely and effectively.


Equipment Needed


  • Appropriate size cannula (18G-24G)
  • Tourniquet
  • Alcohol/chlorhexidine swabs
  • Sterile gauze
  • Transparent dressing
  • Saline flush
  • Gloves (non-sterile)
  • Sharps container
  • Tape

    Procedure Steps


    Preparation

  • Verify patient identity
  • Explain procedure and obtain consent
  • Perform hand hygiene
  • Gather equipment
  • Position patient comfortably

  • Vein Selection

  • Apply tourniquet 10-15cm above site
  • Identify suitable vein (avoid joints, previous sites)
  • Palpate for bouncy, refilling vein
  • Release tourniquet while preparing

  • Insertion

  • Reapply tourniquet
  • Clean site for 30 seconds, allow to dry
  • Don gloves
  • Stabilize vein with non-dominant hand
  • Insert cannula at 15-30 degree angle, bevel up
  • Advance until flashback seen
  • Lower angle and advance 2mm
  • Hold stylet, advance cannula
  • Release tourniquet
  • Apply pressure above cannula tip
  • Remove stylet into sharps container

  • Securing

  • Connect extension set/flush
  • Flush with saline (check for swelling)
  • Apply transparent dressing
  • Document date, time, gauge, site

  • Troubleshooting


  • **No flashback**: Reposition, try different angle
  • **Flashback but won't advance**: May have gone through vein
  • **Swelling during flush**: Remove immediately (infiltration)

  • Complications


  • Infiltration/extravasation
  • Phlebitis
  • Hematoma
  • Infection
  • Air embolism (rare)
